Monitoring and Scoring a Classroom Support Plan

Printable Scorecard

Click PDF to view and print a copy of the Classroom Support Plan. The PDF includes a weekly monitoring scorecard with space to collect up to two weeks of data.

Monitoring focuses on two areas:

• Student Outcome: Did students meet the targeted expectation during the selected routine?
• Staff Implementation: Was the selected strategy implemented during the routine?

Score Today's Data

To enter data for a single day, click Score Today in the upper-left corner of the plan.

1. Enter the student outcome score.
2. Enter the staff implementation score.
3. Add comments if needed.
4. Click Save Score.

Score an Entire Week

To enter a full week of data, use the This Week's Scoring section.

1. Enter scores for Monday through Friday.
2. Add daily comments if needed.
3. Click Save Week when finished.

By default, This Week's Scoring displays dates from the current week.

To enter data for a previous week, click Score a Different Week, select the desired week, and complete the scores as needed.

Tip: Consistent monitoring helps teams determine whether students are meeting the expectation and whether the selected strategy is being implemented as intended. Reviewing both outcomes together can help guide future adjustments to the plan.

Click Graph to view progress data for the Classroom Support Plan.

The graph displays:

• Student progress Scores showing progress toward the targeted expectation.
• Teacher Implementation Scores showing the consistency of strategy implementation.
• Average Ratings for both student outcomes and staff implementation.
• Trendlines to help visualize patterns and progress over time.

Reviewing both student outcomes and implementation data together can help determine whether changes in student behavior are associated with consistent strategy implementation.
