Minute-by-Minute Data Collection Form

This simple, powerful tool lets educators and support staff collect frequency and duration data throughout the school day. No timers, apps, or technical skills required. It pairs directly with Behavior Advantage's Frequency and Duration Tool, so once the day's data is entered, it's automatically graphed for easy analysis and ongoing progress monitoring.

How it works:

• The form displays one box for every minute from 7:00 AM to 4:00 PM, organized into 30-minute columns for quick visual scanning
• Identify one or two target behaviors to track
• When a target behavior occurs: note the time, support the student per their BIP, and once the behavior ends, draw a line across the corresponding minute boxes to mark when it occurred
• Repeat throughout the day as needed
