Functional Assessment Checklist for Teachers and Staff (FACTS)

A brief, semi-structured interview that jumpstarts a functional behavior assessment.

The FACTS is completed with the people who know the student best - teachers, family members, clinicians - and takes just 15–30 minutes. It's designed to get a team from "we're not sure why this is happening" to a clear, testable hypothesis quickly, with efficiency improving each time it's used.

The interview has two parts:

  • Part A: Routines Analysis — pinpoints which routines problem behavior does (and doesn't) show up in
  • Part B: Functional Behavior Assessment — builds a Summary of Behavior that identifies the function of the behavior, along with the specific antecedents that trigger it and consequences that maintain it

The more detailed the Summary of Behavior, the more targeted the intervention that follows, so it's worth taking the extra few minutes to get specific. Confirmatory observations are recommended after the interview to validate what was reported.
