Basic Information
Learn how to complete the Basic Information section of an FBA/BIP, including report details, team members, target settings, and implementation information.
Report Name
Enter a name for the FBA/BIP and click Save. This field is required and must be saved before additional sections can be completed.
Report Date
Defaults to the date the report was created but can be updated if the plan is completed later. This date appears in the header of the PDF report.
Reason for Referral
Briefly describe who requested the plan, the concerns being addressed, the team members involved, and the types of supports or recommendations provided.
Example: North Union High School referred Adam for a functional behavior assessment (FBA) and behavior intervention plan (BIP) within his educational placements. Record review and staff interviews detail a history of interfering behaviors that have impeded Adam’s ability to learn and access curriculum. These behaviors have included: aggression and work refusal. Preventative recommendations and positive behavior supports are provided. The following report is based on direct observations over three days, clinical interviews, record review, and other functional behavior assessments.
Next Team Meeting
Enter the date of the next team meeting. This date will appear as Next Meeting in the header of the PDF report.
Assessment Dates
Enter the dates of when direct observations and/or interviews were conducted to gather assessment data. Additional lines become available after entering up to three dates and clicking Save.
Contributors
List the names and roles of team members who contributed to the assessment and planning process. Additional lines become available after entering up to three contributors and clicking Save.
Tip: Include both the name and role of each contributor (e.g., Mr. Alley, Math Teacher).
Student Information
Click to expand this section and view the student information saved when the FBA/BIP was created. Changes made later in the student's file (e.g., grade level or school) will not automatically update within the FBA/BIP, helping preserve an accurate historical record. Information can be updated here as needed.
