Direct Observation A-B-C Recording Form

Real-time data on what's happening before, during, and after the behavior.

An ABC Chart is a direct observation tool for capturing what's actually occurring in a student's environment as it happens:

  • A – Antecedent: what happened immediately before the behavior
  • B – Behavior: what was observed
  • C – Consequence: what happened immediately after

Recorded across several observation sessions, ABC data helps a team spot patterns, form a hypothesis, and confirm the function maintaining a problem behavior. It also works well retrospectively — as a shared reference when a team is comparing notes on behaviors they've each observed over time and across different settings.
