Assessment Resources
Learn how to document assessment information from observations, interviews, record reviews, and other sources that support the Functional Behavior Assessment.
The Assessment Results section provides space to summarize how the Functional Behavior Assessment (FBA) was conducted and document the findings. Each subsection is optional and may be completed based on the scope and complexity of the assessment. Any section left blank will not appear in the FBA Report PDF.
Assessment Results
Summarize the overall findings of the FBA, including the identified function(s) of the Target Behavior(s), important behavioral patterns, and key conclusions drawn from the assessment.
This section may be used to:
- Provide a concise summary of the entire FBA.
- Supplement the detailed information entered in the sections below.
- Briefly describe how the assessment was conducted when a shorter report is appropriate.
Leave this section blank if the assessment findings are already adequately summarized in the detailed sections below.
Background Health & Medical Factors
Summarize any relevant health, medical, developmental, or mental health factors that may influence the student's learning, behavior, or response to intervention.
Examples may include medical diagnoses, medications, sleep concerns, sensory needs, developmental history, mental health conditions, or other health-related information that provides important context for understanding the Target Behavior.
Direct Observations
Document when and where observations occurred, who conducted the observations, and the settings in which they took place.
Summarize the student's behavior, antecedents, consequences, environmental conditions, and any patterns observed across multiple observations, such as:
- Time of day
- Activities or routines
- Staff responses
- Peer interactions
- Environmental factors
Clinical Interviews
Summarize interviews conducted with teachers, parents/caregivers, related service providers, and, when appropriate, the student.
Include information regarding:
- Frequency, intensity, and duration of behaviors
- Common triggers and maintaining variables
- Previous interventions
- Effective supports
- Communication, social, emotional, and adaptive functioning
Record Review
Summarize records reviewed as part of the assessment process and any findings that contributed to understanding the student's behavior.
Examples may include:
- Individualized Education Program (IEP)
- Psychoeducational evaluations
- Academic records
- Previous FBAs or Behavior Intervention Plans
- Related service reports
- Health or medical records
- Attendance and discipline records
- Progress monitoring or behavior data
Include any relevant findings that helped identify behavioral patterns, skill deficits, environmental factors, or possible functions of the Target Behavior.
Analysis of Effective & Ineffective Interventions
Summarize interventions that have previously been implemented and describe which strategies have or have not been effective.
When appropriate, include possible reasons why interventions succeeded or were less effective, such as consistency of implementation, alignment with the function of behavior, environmental supports, or reinforcement practices.
Functional Assessment Procedures
Document any standardized assessment tools or structured procedures completed as part of the FBA.
Examples include:
- Functional Assessment Screening Tool (FAST)
- Functional Assessment Checklist for Teachers (FACTS)
- Motivation Assessment Scale (MAS)
Include who completed each assessment, when it was completed, and a brief summary of the results and how they contributed to identifying the hypothesized function(s) of the Target Behavior.
